Dr. Sam Forrest (she/they) is proud to be practicing in her hometown of Bozeman, Montana. She earned her Bachelor of Science in Neuroscience from Bates College in Maine in 2013, where her early work in crisis and sexual assault counseling sparked a deep interest in trauma-informed clinical care. After returning to Montana, she worked as a crisis counselor on the state suicide hotline and also as a medical assistant specializing in sexual and reproductive health, HIV counseling and testing, and gender-affirming care. These experiences inspired her to complete a Master of Health Science (MHS) at Montana State University. She focused her studies on gender health, nutrition, and health equity, and graduated in 2017.

Dr. Forrest earned her Doctor of Medicine (MD) from the University of Washington School of Medicine in 2022. She went on to complete her Family Medicine Residency at Kaiser Permanente in Seattle, Washington, where she served as Chief Resident and became board certified in Family Medicine in 2025. She plans to become board certified with the American Board of Obesity Medicine in late 2025.

Dr. Forrest practices with a weight-inclusive, trauma-informed, and holistic approach. She prioritizes a safe, nonjudgmental space where patients feel empowered and supported in their health journeys. She is passionate about building trusted partnerships and providing care that respects each person’s unique experience. During her residency, Dr. Forrest completed specialized training in hormone therapy for all genders, menopause management, women’s health, HIV care, preventative medicine, mental health, dermatology, and LGBTQ+ and gender-affirming care. She’s thrilled to provide full-spectrum care — from newborns to older adults. She offers a wide range of procedures, including newborn circumcision, vasectomy, IUD placement with multiple pain relief options, colposcopy and annual skin exams.

Dr. Forrest believes that strong relationships, joyful movement, restful sleep, and nourishing food are all essential to physical and mental well-being. Outside the clinic, you are most likely to find her trail running with her two goofy golden retriever mixes while listening to a podcast, Nordic or downhill skiing with her partner, or sharing outdoor adventures and sweet treats with friends and family.
