Bozeman Creek Family Health accepts the following major health insurance plans: Blue Cross/Blue Shield of Montana, New West, Allegiance, and United Health Care.  Unfortunately, we are not accepting any new Medicare and/or Medicaid patients at this time.

If you have a health plan in addition to those listed above, we will still bill your insurance company directly for reimbursement.  You will be responsible for any remaining fees after your insurance carrier has paid their share.  If you have any questions, please contact your insurance company directly for information regarding your specific plan coverage.

Most health plans require an office co-payment, which we will collect from you at the time of service.
