Bozeman Creek Family Health will accept assignment on the following health plans: Blue Cross/Blue Shield of Montana and New West.  This means that we will bill these entities for any fees that we would charge you for services we provide and have agreed to accept their allowed amount.  Most health plans also require a patient deductible and co-payment, which we will collect from you at the time of service. 

If you have a health plan that is not on this list, we will charge our regular fee, and as a courtesy to you, bill your health plan directly for reimbursement.  You will be responsible for any fees remaining after your insurance carrier has paid their share.
