To our new and existing patients, welcome to our website and BCFH family, our staff is honored to be a part of your healthcare.  Listed below are new patient forms that you are welcome to complete before your appointment and can be either mailed, emailed, faxed or brought to the office for your scheduled visit.

If you are unable to complete the forms before your appointment, they will be available at the front desk when you check in for your appointment.  We ask that you arrive 5-10 minutes before your scheduled time to complete the required admin paperwork.

*Please also bring a copy of your insurance card and photo identification (drivers license, military ID, etc)
